ABSTRACT

The mentor system has been applied in the standardized residency training, currently the "one to one" ortwo mentors′ system are more common applied. However, the clinical training of general practitioners needs joint effort of mentors from the general practice department and specialists in hospital, as well as of mentors from primary health institutes. Thus, we proposed a "three-in-one" tutorial system, which consisted of one GP mentor, one specialists in the hospital and one primary physician for residency training of general practice. The implementation of the "three-in-one" system has motivated the enthusiasm of specialists and community instructors, and enhanced their responsibility. The mentors from different department can learn from each other and complement each other, so the overall teaching level can be raised and the quality of standardized training of general practitioners can be improved.

ABSTRACT

Objective@#To study the current situation of humanistic quality education among residents standardized training students in Sichuan Province and analyze the effect, so as to provide references for the enhancement of humanistic quality education for these students.@*Methods@#Five 3A hospitals in Sichuan Province were randomly chosen as the object hospital, and 15 administrators, 500 teachers, 500 students and 300 patients were included. From the aspects of cognition, opinion and suggestion on humanities quality education, a questionnaire survey was conducted. SPSS 19.0 was used to analyze data.@*Results@#The humanistic quality education among students in these hospitals were not good. Among students, 43.1% believed that humanities quality education received deficient attention, and 71.7% thought that curriculum could not meet the training requirement; among teachers, 54.1% thought that teaching ability needed to be strengthened and 60.9% considered that the current teaching conditions and methods were not good enough.@*Conclusion@#Effect of humanistic quality education is not good and we suggest solving the problem through improving humanistic management, teachers’ construction, humanistic courses and assessment system.

ABSTRACT

ABSTRACT

Objective To investigate the application of "medical Salon" teaching in standardized training of residents in gynecology and obstetrics department. Methods A total of 30 residents of obstetrics and gynaecology receiving standardized training in the People's Hospital of Wuhan University from August 2015 to March 2018 were enrolled. They were randomly divided into experimental group and control group, with 15 persons in each group. In addition to daily training, the experimental group held a "theoretical salon" once a week and a "surgical salon" every quarter. Both groups of physicians took examinations of basic theories and clinical practice level and received evaluation of training satisfaction;SPSS 17.0 was used to perform t-test or paired t-test on intra-and inter-group data. Results Before the standardization training began, there was no significant difference in the scores of the basic theory and clinical practice between the two groups (P>0.05). After the training, the basic theory and clinical practice scores of the two groups were improved compared with those before training, and the difference was statistically significant (P<0.05). After the training, the scores of the basic theoretical examination of the experimental group were higher than those of the control group [(95.20±3.67) vs. (82.10±5.62)], and the scores of clinical practice of the experimental group was higher than the control group [(96.80±3.26) vs. (79.50±5.28)]; the teaching satisfaction scores of the experimental group were higher as well [(95.50±3.54) vs. (86.70±4.95)]. The differences were statistically significant (P<0.05). Conclusion The"Medical Salon"teaching helps to improve the quality of standardized training for obstetrics and gynecology residents and is worth promoting.

ABSTRACT

Objective To get well-informed of the practical skills examination of residents' standardized training in order to detect problems and improve the quality of training. Methods A total of 4484 clinical medical examinees from 2010, 2013 and 2017 in Liaoning province were investigated by questionnaire, from which 4412 valid questionnaires were collected and the total survey efficiency was 98.39%. The survey included the standardized patient (SP) examination, the small station test, the computer simulated case (CCS) examination and the clinical skill operation test station with parts 20 projects. Excel was used to sort and record the data. SPSS 10.0 software was used for statistical analysis. Results All parts of the practical skill assessment was reasonable. The examinees' satisfaction with the test is showing an upward trend year by year, up to 73.93%in 2017. In the standardized patients examination, the key points of the role play (including the fidelity and simulated authenticity of the physical signs) need be strengthened (14.84% in 2010). Examinees thought the difficulty of computer simulation case examination is increasing year by year. More than 2/3 examinees evaluated the clinical skill operation test station set was reasonable and the difficulty was moderately. Conclusions We should further deepen the exploration of standardized patients assessment model; strengthen the medical humanities in training; improve the residents' clinical thinking;take examination as the guidance and standardize clinical training.

ABSTRACT

Objective To investigate the implementation effect of standardized training of residents in obstetrics and gynecology.Methods A total of 40 students who participated in the standardized residency training of obstetrics and gynecology in our hospital from 2016 to 2018 were selected as respondents.The scores of professional dedication,sense of responsibility,communication skills,document writing,medical history collection,physical examination,diagnostic and differential abilities,drug use,clinical skills,and computer use were evaluated before and after training.The scores of theoretical and skill exams before and after training were collected and compared to evaluate the implementation effect of standardized training of residents in obstetrics and gynecology.Results After training,there were significant changes in the scores of professional dedication [(4.775 ± 0.366) vs.(7.475 ± 0.362),P<0.05)],sense of responsibility [(4.800 ±0.333) vs.(7.850 ± 0.366),P<0.05],communication skills [(4.575 ± 0.382) vs.(7.550 ± 0.339),P<0.05],document writing [(4.833 ± 0.376) vs.(7.762 ± 0.343),P<0.05],medical history collection [(4.762 ± 0.314)vs.(7.738 ± 0.316),P<0.05],physical examination [(4.434 ± 0.347) vs.(7.565 ± 0.317),P<0.05],diagnostic and differential abilities [(4.239 ± 0.274) vs.(7.348 ± 0.314),P<0.05],drug use [(4.370 ± 0.261) vs.(8.022 ±0.246),P<0.05],clinical skills [(4.370 ±0.330) vs.(8.217 ±0.265),P<0.05],and computer use [(4.652 ± 0.260) vs.(8.630 ± 0.289),P<0.05].There were also significant changes after training in the scores of theoretical exam [(70.950 ± 1.165) vs.(89.675 ± 1.164),P<0.05] and skill exam [(67.575 ± 0.947) vs.(88.150 ± 0.877),P<0.05].Conclusion Standardized training of residents in obstetrics and gynecology can enhance their clinical skills and improve their clinical thinking ability.

ABSTRACT

Objective To investigate the current situation of empathy and self-efficacy, and relation with perform-ance of objective structured clinical examination in residents of standardized training. Methods Questionnaire sur-veys with Jefferson scale of empathy health professionals and general self-efficacy scale were conducted among resi-dents of grade 2015 and grade 2016 from department of internal medicine of Peking Union Medical College Hospital (PUMCH). Results Totally 101 questionnaires were delivered,and 99 were collected back. The average empathy score was 111.3±1.2,with that of grade 2015 slightly higher than that of grade 2016. The empathy score from resi-dents of different degree and different sources showed no significant difference. The average self-efficacy score was 22.77±0.50. The score of residents of grade 2015 was significantly higher than that of grade 2016. The score was higher in residents with higher degree. The score of residents from PUMCH was higher than the other subgroups. The score of empathy showed no significant correlation with OSCE scores, while the score of self-efficacy of resi-dents of grade 2015 significantly positively correlated with scores of medical recording (R=0.35,P<0.05),case analyzing (R=0.31,P<0.05) and average score(R=0.33,P<0.05) of OSCE. Conclusions The empathy and self-efficacy of residents remained to be improved, and could be improved through clinical training. Psychological evaluation could be inducted into standardized resident training system,and provide helpful supplementary to OSCE with more comprehensive evaluation of residents.

ABSTRACT

Objective To analyze the results of theoretical test and clinical skills examination of standardized resident training gradua-tion exam in Beijing in recent years, to improve the examination standards and assessment models. Methods The results of theoretical test and clinical skills examination from 2011 to 2016 were collected. The difficulty, degree of differentiation, and factors related with the result of clinical skills examination were analyzed. Results The test paper is difficult with good degree of differentiation from 2014 to 2016. Edu-cation degree and year were not related with the clinical skills (P>0.05), while the hospital level was (P<0.05). Conclusion The standardized training and assessment of residents in rehabilitation medicine need continuously attention. Combined with the results of the assessment and the problems exposed in the assessment, a feasible scheme should be put forward, in order to improve the quality of resident training.

ABSTRACT

Objective To survey the satisfaction degree of the postgraduates in professional degree with The Joining Together of Double-Track education model in the current stage of professional degree graduate education and standardized training of residents in China. Methods According to various factors, such as the current situation of postgraduates in medical universities, we sought the opinions of relevant experts to design questionnaire. Meanwhile, to enhance the reliability of the questionnaire and the survey, we chose the postgraduates of Shengjing Hospital of China Medical University first to do the pre-survey, and according to the feedback, we adjusted part of the aspects, thus formed a formal questionnaire, which included the satisfaction with training of clinical practice ability, training of research ability, and tutors' assessment etc. Finally, the Chinese New Youth Forum online released the questionnaires, selecting the postgraduates in professional degree who were participating in, or had participated in the completion of the standardized training as the participants, which took place between March 2016 and May 2016. SPSS 16.0 was used for statistical analysis. The evaluation results of different majors were tested by Kruskal-Wallis rank sum test. Results According to the results of the survey, the aspects in the clinical resident standard-ized training that the 1000 postgraduates were more satisfied with were as follows: training time [42.8%(n=428)], training center [41.8% (n=418)], training of clinical practice ability [41.6% (n=416)], tutors [40.2%(n=402)], economic income [38.8%(n=388)], department arrangements [38.4% (n=384)], training of research ability [37.5%(n=375)]. There is a significant difference in the satisfaction degree of different pro-fessional graduate students in theJoining Together of Double-Trackeducation model (P<0.05). ConclusionThe Joining Together of Double-Track education model should be compatible with the training objectives of postgraduates in professional degree. Much more attention should be paid to the post-graduates, satisfac-tion degree with the clinical resident standardized training, as well as the requirements during the training period, improve the evaluation of graduate students' ability of scientific research, econo-mic income and so on, so as to improve the training system for the postgraduates.

ABSTRACT

Standardized rotary residency training is an important part of clinical medical education. Traditional clinical teaching can't meet the rapid development of oncology medicine. In the rotary residency training in oncology department, we put forward the problems encountered in clinical practice, stimulate the interest and initiative of residence, the PBL teaching model is combined with the evidence-based medicine in the teaching process through the relevant training, literature review and discussion. By standardizing the treatment concept the residence's understanding of the basic theory and frontier knowledge of oncology was improved, the thinking innovation and clinical practice ability of the residency doctors were enhanced.

ABSTRACT

Objective To explore the application of the evidence-based case report method in the standardization training of resident physicians in the department of endocrinology. Methods Fifty-one resi-dents recruited from the endocrinology department were randomly divided into the experiment group (26 cases) and control group (25 cases). The evidence-based case report method was conducted in the ex-peri-mental group, while the traditional clinical teaching mode was applied in the control group. The re-sults of the examination of two groups including the theoretical test and skill test were compared and the question-naire survey was conducted to find out the differences of students' interest in learning, clinical skills and satisfaction. All data were compared using t test. Results There was no significant difference in theoretical test between the two groups (P>0.05). The total score of the skill test in the experimental group was (85.13 ±4.63) and the control group was (76.79 ±3.11) (P=0.000). The students in the experimental group showed that the teaching method had more significant effects on improving their learning interest and initiative, clinical thinking ability, clinical work ability, literature review ability and medical article writing ability. Scores of satisfaction with teaching methods (total score was 10) for experimental group were (9.69 ±0.34), which was also significantly higher than those in the control group (8.01 ±0.39) (P=0.000). Conclusion The application of evidence-based case report teaching method in the standardized training of resident physicians in department of endocrinology is worthy of being perfected and further popularized in clinical teaching work.

ABSTRACT

To push on the process of the effective link between graduate education for profes-sional master degree of oral medicine and resident standardized training, a professional curricular con-struction was explored. The system contents included drawing up teaching program, writing reference books and building examination question bank. To realize the objective that combine theory study and clinical practice for graduate education and resident standardized training, the main courses were des-ignated for base professional knowledge, clinical operative skills and research progresses of oral medi-cine, by making multimedia courseware and lectures video frequency.

Objective To analyze the feasibility of the policy that “Residents service or employment at primary care institutions after standardized training”.Methods Analyzing the feasibility of this policy by using questionnaire survey and in-depth interview.Results Constituent ratios of overall feasibility were 58.3% (532/912)and 66.1%(603/912)for specialist and general practitioner respectively; percentage of feasibility of in-depth interview was 46.7% (7/15).Conclusion The policy has feasibility,if government formulates and implements perfect complement policies effectively,and makes this policy to be impassable stage to resident.

ABSTRACT

Policy background and stakeholder theory were elaborated.It could be concluded that the key stakeholders of the policy were residents,training hospitals,healthcare administrators,primary healthcare institutions and patients of such institutions.Through analysis of these stakeholders,effects on the interests of all parties were investigated and strategies to improve feasibility of the policy were put forward.Two key points were suggested for such policy goals.First,top-level policies should be designed and be effectively implemented.Second,conversion of the residents from permanent hospital employees to “contractors”or“ freelancers”.

ABSTRACT

Problem-based learning (PBL)was used in resident standardized training in depart-ment of pediatrics of Changhai Hospital. Attending doctors with authority were taken as leaders teach-ing group and 3-5 resident doctors as team members. Cases were set up according to the targets of resident standardization training and common clinical diseases in each system. According to the results of the questionnaire after the teaching , both teachers and students were satisfied with the teaching effect and expected targets were reached. Residents made great progress in handling clinical problems.

ABSTRACT

As one of the first resident standardized training bases, department of ophthalmology of Shanghai Ruijin Hospital participated in this reform process from 2010. Relevant rules and regulations (training management system , training scheme implementation system and evaluation system ) were strictly obeyed. When new problems emerged, under the guidance of department in charge, a series of regimens were formulated and improved gradually by Ophthalmology Professional Committee of Shanghai Resident Standardized Training Department. Based on reviewing and summarizing the work in the last 3 years, some thoughts and suggestions on the resident standardized training in future were put forward ,in-cluding how to better solve the“heavily used, lightly cultured” problem, the“disregarding medical ethics establishment”problem, the“disregarding assessment of teachers”problem and the“disregard-ing obtaining employment”problem.
